On the fifth line the word after 'den' is 'pamatnij'
"w Strzedu den Pamatkij Swatyi Fabia,,
na Ssebestijana"
"v Středu den Památky svatí Fabia,,
n Sebastian"
St. Fabian and St. Sebastian feast day = (January 20th)

(P.P. = Pana Pana) = by the Noble Lord MasterVery likely because in all examples I found it precedes a name of a person.
Down where it says the year from the birth of Christ - the year is written as Sestnaztisteho sestnazteho - is this year 1616?
At the end of the third line there is a work 'zdibi' - this could be a small, but very old town next to Prague called today Zdiby. I could be wrong about that one.
Opprstorff = Oppersdorff noble family

Czastalowisych = Častolovice
Tynissti = Týniště nad Orlicí
The words are probably 'z dubu'. The scribe most likely connected the two.
1616 seems correct. I have a hard time seeing a 'c' in front of the 'z'.
